What should I bring to my first appointment?

For your first appointment, please bring your photo ID, insurance card, a list of current medications (including dosages), your medical history information, any relevant medical records or test results, and a list of questions you may have for your provider. Arriving 15 minutes early to complete paperwork is recommended.
